Unfortunately there are some dental insurance plans we don't accept, including Medicaid, Healthplex, HMOs, DMOs, and others. If we don't take yours, we'll content you ahead of your visit.

NEW PATIENT FORMS

Once your appointment is scheduled, our team will send you new patient forms via email. Make sure you fill out our new patient intake forms before your visit in order to speed things up when you’re in the office. You can send them to us online or print them and bring them with you on your visit. If you need help with the forms, just show upon about 20 minutes early for your appointment and well get you squared away!

PAYMENT

We happily accept all major credit cards, and we work with many PPO dental insurance plans. Contact us before your visit to verify your insurance. Flexible 0% interest payments plans and 3rd party financing available for everyone!
